Submit laboratory, field-sampling, courier or specialty-service capabilities for current and future 516Labs programs.

Laboratories

Include the performing location, major testing capabilities, accreditation or certification scope, matrices and methods, service geography, standard and rush turnaround, receiving hours or cutoffs, pickup coverage, bottle or kit charges, minimum charges, volume pricing, reporting or EDD options, and billing terms.

Field sampling providers

Include service geography, certifications, supported sample types, normal scheduling lead time, rush or after-hours availability, and trip or mobilization charges.

Couriers and logistics providers

Include service geography, scheduled and same-day coverage, operating hours, custody procedures, pricing structure and relevant specimen or environmental-sample experience.
